Hacking can be done different ways, by installing a keylogger for example. Measures you can take to protect your account:
#1 first of all change your security question and secret answer. To change or update your Password Question and Secret Answer, send a mail to: my-login-request@yahoo-inc.com.
For security reasons, Yahoo requests that you send this information from the Yahoo! Mail account associated with the Password Question and Secret Answer. For the Subject of the email, type: Password Question and Secret Answer.
In the text of the email, include:
Your Yahoo! Mail ID.
The ZIP code you gave when you registered for the account (non-U.S. residents, please include country).
Your birthdate.
The alternate (non- Yahoo!) email address listed for your Yahoo! ID.
Your current Password Question and Secret Answer (exactly as you entered them when you registered).
Your new Password Question and Secret Answer.

#2 Change your password! Click “my account” at the top of this page, enter your current password, click “continue”. Click the “change password” link near the top. Once again enter your current password. Type in the new one, confirm by typing it again. Don’t forget to save the new settings! Note: your new password is valid for all personalized Yahoo services like YA, messenger, e-mail…

If you suspect that somebody may have installed a keylogger on your system, get the freeware tool “hijackthis”, it quickly scans your computer to find settings that may have been changed by spyware, malware or other unwanted programs. HijackThis creates a report, or log file, with the results of the scan. Check the “overview, FAQ and quick start guide” buttons in the top bar to find out how this tool works.

Then google for “hijackthis forum”, click on one of the many links and follow the prompts. Those forums have volunteers who help you to analyze your hijackthis log and get rid of the malware on your computer. Below are a couple of links to such forums:
